Whether it’s a planned check-up or a surprise visit to the Health Center, Campminder makes it easy to stay on top of medical documentation.

Add a Treatment
For any unscheduled visits, you can search for the person in the health center and then add in a treatment/visit as needed.
Find the person's record by entering part of the name in the Health Profile search bar. In the record window, click on the "Treatment" button.
If you're entering a visit after it happened, you can adjust the date and time to reflect when the care actually occurred.
As you document the visit, include the camper’s vital signs—temperature, pulse, blood pressure, and respiration. Note who provided care in the MD/RN field, and where the visit or incident took place using the Location field. Use Disposition to record the outcome of the visit, and indicate whether parents were notified.
Add any symptoms under Presenting With, and if applicable, click a treatment from the Frequent Treatment list. In the Treatment Details section, describe the care given, including actions taken and any instructions provided.
If a follow-up is needed, check the Follow-Up box and enter the next visit’s date and time.
Note: Once a treatment is saved in Campminder, it can’t be deleted or edited. If you need to make changes, just add a new entry.
Click the purple "Save" button once you've entered all information.
Add a Log
Logs are expected visits to the medical center. Generally, you would use these to record check-ins and follow-up visits.
Find the person's record by entering part of the name in the Health Profile search bar. In the record window, click on the Log button.
If you're logging a visit after it happened, you can adjust the date and time to match when it actually took place.
Record the camper’s vital signs—temperature, pulse, blood pressure, and respiration—and select the staff member who treated them from the MD/RN drop-down.
Choose a Log Prefix to categorize the visit, such as “Check-In” or “Follow-Up.”
Use the Comments box to describe what happened during the visit, including any observations or actions taken.
Click the purple "Save" button once you've entered all information.
MAR
In the Health Profile’s MAR section, view medication disbursements for today or any selected day for quick, easy administration.
Find the person's record by entering part of the name in the Health Profile search bar. In the record window, click on the "MAR" button.
At the top, you can set the date, and then filter by Planned, Unplanned, or Both
Planned: any medication (whether prescription or OTC) listed in a person's lower medical tab to be taken on a specific schedule (or even As Needed).
Unplanned: any medication is given not at the time it was scheduled to be given, or an OTC medication not listed that the person takes on a specific schedule.
Administer Planned Medication
Click a purple "Administer" button to record when a prescription or OTC med was given.
Enter the dosage given at the time—if you’re unsure, you can double-check listed medications and dosages on the right side of the Health Profile.
The date field is fixed to today when administering meds, but you can exit this window and select a different date from the top of the Medication tab if needed.
Set the time the medication was given, and add any important details in the Note field—these will be included in future reports.
Administer Unplanned Medication
Click Administer Unplanned to record medication given outside the usual schedule, such as an unexpected OTC dose.

Note: you can only choose from the radio buttons indicating a medication existing in the camper's record, including Over The Counter.
Add Medical Logs and Treatments in the UPR (Unified Person Record)
Instead of going to the health center, you have the ability to record treatments and logs from a camper or staffer's record.
Add a Treatment In a Record
Navigate to the Medical tab on the lower half of a staff/camper's record.
Click on the Log & Treatment tab. If there are previous entries, they will appear here.

Click the gray Add Treatment button. A window will open. The Date and Time will start with today's date and time, but you can edit it to record when the visit happened.
In the 'Presenting With' box, describe why the camper needs treatment and their injury or illness details.
Note: Entries cannot be edited or deleted once submitted.
The Frequent Treatment box shows treatments you set up in your medical settings. Click a treatment to highlight and apply it.
Use the Treatment box to note what care was given, including actions and instructions.
Select the provider from the MD/RN drop-down and the visit location from the Location drop-down.
Choose a Disposition, note if parents were notified, and check Follow Up if needed.
When complete, click the green Save button.
Add a Log In a Record
Navigate to the medical tab on the lower half of a staff/camper's record.
Click on the "Log & Treatment" tab. If there are previous entries, they will appear here. To view full details of any previous entry, click on ''View Link.''
Click on the gray ''Add Log Entry'' button.
A pop-up will appear with the Date and Time defaulted to now. You can adjust this to reflect when the visit occurred.
In the Comments box, enter detailed notes about the visit—thorough documentation supports camper care and meets insurance and state requirements. Note: entries cannot be edited or deleted once saved.
Select the staff member who saw the camper from the MD/RN drop-down, and enter the log prefix.
Note: The entry date reflects when the information is added to the system. You typically won’t need to change it—but you can if needed.
The name of the person logged in will appear under Entered By. You may also change this name.
Click the green Save button.
